DCYF Waiver Request Form
The purpose of this form is to identify DCYF Core Academy courses that a new DCYF staff member may permanently waive due to past experiences or qualifications. The criteria that qualify for a permanent waiver are up to the discretion of the DCYF staff member鈥檚 supervisor, DCYF leadership, and CWEP. This form will not be used for temporarily waiving a course. This form is for internal use only and will not be shared with anyone outside of CWEP without the consent of the DCYF supervisor.
Please complete this form to submit a request for the following:
- A new curriculum or update to existing curriculum
- Training logistics support, such as scheduling a training and creating registration forms
- Submitting a roster for a completed training for training credit on behalf of a group
